Dear blood donors,

To be in-line with AABB(American Association of Blood Banks) requirements, HSA has amended parental/guardian consent form to include the donation process with effect from  16 November 2009. Please refer to HSA Website or CLICK HERE for details.

In Singapore, blood donors who are 16 and 17 years old are required to present this form before each donation.

Papermonkey Theatre Ltd is a non-profit theatre comany formed in April 2008. It is presenting an interactive life size puppet show “HUG THE TREE” at the Republic Cultural Centre, Repblic Polytechnic from 17-20 November 2009. You may get more information HERE.

Papermonkey is offering 30 free tickets for the 8pm show on 20 November 2009 to blood donors who have children 4 years and above. Please call us at 62200183 to register, on a first come first serve basis.  Each donor may request up to 4 tickets.
